After a hospital stay
Home from the hospital? Thirty days of check-ins
If your clinic enrolls you after a hospital stay for heart failure, your daily check-in changes for 30 days. Each morning you answer six short questions and weigh yourself. Your care team is told when your answers show a change.

Send your discharge papers
Take a photo of each page, or upload the PDF. A nurse checks what MyndfulPal reads from them before anything is used, and only your clinic’s care team can open them.

Six questions each morning
Your breathing, any swelling, your weight, your medicines, anything new, and your blood pressure. Each answer saves as you go. Cuff not handy? Tap Remind me later and we’ll remind you in two hours.

Told what to do, right away
If you report chest discomfort, the app tells you to call 911 if it’s happening now, and your care team is alerted. Much more short of breath than usual? It asks you to call your clinic today.
Your Today screen shows which day of the 30 you’re on. After that, you go back to the usual check-in.
Companion
An AI to talk to, any hour of the day
Tell it how you’re doing, ask a question, or just talk. Type, speak out loud, or tap a picture when words are hard. It can suggest a short guided exercise, like a 3-minute seated stretch.
Your chats are private. Your care team gets a short daily summary, and an alert if you might be unsafe. You’ll see this before your first chat.
The companion is an AI. It doesn’t replace your care team, and it isn’t for emergencies.
Wearables
Bring the wearable you already have
Connect it in a couple of taps. Over the first few weeks, MyndfulPal learns your usual heart rate and sleep. If something looks off, it asks you what’s going on, so your care team hears it from you.

Disconnect any time from the Me tab. MyndfulPal deletes its access to your account when you do.
Your medicines and your progress, in your pocket
Reminders at your times
Your care team sets up your medicines. You get a reminder at each dose time and tap Taken. Reminders never name your medicines, so they’re safe to show on a lock screen.
See how your week went
The Progress tab shows your check-ins, mood, blood pressure and doses by week or month. Your care team sees the same numbers you do.
Getting started
Ready in a few minutes

Open the invitation
Your clinic sends it by email. Open it on your phone and tap the link.

Enter your code
We email you an eight-digit code. That’s how you sign in each time, with no password.

Confirm it’s you
Enter your date of birth so we know the invitation reached you. Then read and agree to how MyndfulPal works.

Choose your reminders
Pick when you’d like to check in, and read exactly what your care team will see.

Add it to your home screen
On iPhone, tap Share, then Add to Home Screen. On Android, tap Install app.
